“The Canadian Hospital Reporting Project (CHRP) is a national quality improvement initiative from the Canadian Institute for Health Information (CIHI). CHRP’s web-based, interactive tool gives hospital decision-makers, policy-makers and Canadians access to indicator results for more than 600 facilities from every province and territory in Canada.
CIHI selected 21 clinical and 9 financial indicators for CHRP, based on their relevance to performance measurement and quality improvement. The selected indicators measure:
- Clinical effectiveness;
- Patient safety;
- Appropriateness of care;
- Accessibility; and
- Financial performance.
These indicators were chosen after reviewing existing hospital performance indicators and collaborating with experts in the field. The goal of CHRP is to foster quality improvement, learning and action.

“This toolkit is designed to help your hospital understand the Quality Indicators (QIs) from AHRQ, and support your use of them to successfully improve quality and patient safety in your hospital. Created by the RAND Corporation and the University HealthSystem Consortium with funding from AHRQ, it is available for all hospitals to use free of charge. The toolkit is a general guide to using improvement methods, with a particular focus on the QIs.
The AHRQ QIs use hospital administrative data to assess the quality of care provided, identify areas of concern in need of further investigation, and monitor progress over time. This toolkit focuses on the 17 Patient Safety Indicators (PSIs) and the 28 Inpatient Quality Indicators (IQIs).”

“Putting Health in context: What is health? For some, health means the absence of disease and pain; for others, it is a general feeling of wellness. The World Health Organization (WHO) defines health more broadly: ‘the state of complete physical, mental and social well-being and not merely the absence of disease or infirmity.’
This broad definition aligns with the Conference Board’s overarching goal in benchmarking Canada’s performance—to measure quality of life in Canada and in its peer countries. Most Canadians would agree that without health, quality of life is severely compromised.
How do we measure health performance?: To measure health performance, we evaluate Canada and 16 peer countries on the following 11 report card indicators: life expectancy; self-reported health status; premature mortality; mortality due to cancer; mortality due to circulatory disease; mortality due to respiratory disease; mortality due to diabetes; mortality due to diseases of the musculoskeletal system; mortality due to mental disorders; infant mortality; and mortality due to medical misadventures.
It is important to note that the Conference Board is not attempting to rate Canada’s health care system. Although the health care system has an impact on the health status of a population, our goal is to evaluate the health status of Canadians and of their peers in other countries.

The Guide to Reducing Unintended Consequences of Electronic Health Records from the Agency for Healthcare Research and Quality aims to teach healthcare providers how to avoid the risks and pitfalls of electronic health care records.
“EHRs can offer many benefits to health care providers and their patients, including better quality of medical care, greater efficiencies, and improved patient safety. However, even if these benefits are achieved, you will almost certainly face some unanticipated and undesirable consequences from implementing an EHR. Such consequences are often referred to as unintended consequences.
Unintended consequences can undermine provider acceptance, increase costs, sometimes lead to failed implementation, and even result in harm to patients. However, if you learn to anticipate and identify unintended consequences, you will be in a better position to make effective decisions, clarify tradeoffs, and address problems as they arise.”

“This How-to Guide is designed to support hospital-based teams and their community partners in codesigning and reliably implementing improved care processes to ensure that patients who have been discharged from the hospital have an ideal transition to the next setting of care (such as a primary care practice, home care, or a skilled nursing facility).
The Guide includes:
- Getting Started: This section lists steps to get started on creating an ideal transition for patients being discharged from the hospital, a post-acute care setting, or a rehabilitation facility.
- Key Changes: Four key recommendations for improving the transition out of the hospital are described, including typical failures encountered, recommended measures to guide improvement, and tools and resources to help teams implement the changes.
- Testing, Implementing, and Spreading Changes: A review of fundamental improvement methods and resources for testing changes before they are implemented and spread more widely throughout the organization.

“Partnering to Heal is a computer-based, video-simulation training program on infection control practices for clinicians, health professional students, and patient advocates.
The training highlights effective communication about infection control practices and ideas for creating a “culture of safety” in healthcare institutions to keep patients from getting sicker. Users assume the identity of the following five main characters and make decisions about preventing healthcare-associated infections (HAIs):
- A Physician, Nathan Green, Director of a Hospital Post-op Unit, ready to start new prevention efforts in the unit;
- A Registered Nurse, Dena Gray, working to learn effective communications skills that could make the difference for her patients;
- An Infection Preventionist, Janice Upshaw, a new employee charged with using a team-based approach to reducing infections;
- A Patient Family Member, Kelly McTavish, whose father was just admitted to the hospital;
- A third-year Medical Student, Manuel Hernandez, who wants to gain confidence to make a difference for his patients.”
